We continue to closely monitoring federal and state guidance on the spread of COVID-19 and are working to ensure that patients get the care they need. While we are seeing patients in the office, we are trying to limit exposure to our patients, providers and staff. Only the patient will be allowed in our office for the visit with the provider.
*We are only allowing 1 support person (no one under the age of 18) for *OB sonograms only. If you have a support person, they will only be allowed back for the OB sono only; they will also need to pass the screening process to be allowed in the building. Any patient that has a support person will have to wait in their vehicles until the staff calls you on your cell phone to notify that the sonographer is ready.
**If the patient is a minor (under 18 years of age), the parent will be allowed to go back with the patient for the visit. The law requires that a parent accompany a minor for the visit. Both will be screened and both will need to pass the screening process to be allowed back for the visit. The minor and parent will need to wait in their vehicle after being screened, as only patients are allowed to be in the building in the waiting room.
All patients are being screened upon arrival, masks are required to enter the building as well. Please have your completed paperwork, insurance and driver’s license/identification ready prior to entering the building. No food or drinks are allowed in the office. Your temperature will be taken, anyone with a temperature 99.5 or above will be rescheduled and you will also have to answer our screening questionnaire given by our screener. Please allow our screener to ask all the questions and patiently wait for them to complete each question before answering. If you fail the screening process, you will be rescheduled.
Please ensure that you arrive with the required paperwork completed upon arrival, if your paperwork is not completed you risk being rescheduled. Please print off the appropriate paperwork for your appointment. You can bring your completed paperwork, upload it to the patient portal, or fax it to Medical Records at 817-416-3667. Please also be prepared to provide your driver’s license/or ID along with your insurance card for scanning and completed paperwork prior to your appointment. We do not accept electronic copies of your insurance cards, if you have an electronic copy, please upload it through the patient portal, PDF versions only accepted through the patient portal.

You were advised while scheduling your appointment which paperwork to complete, if you have forgotten, you will need to contact our office at 817-416-2229, to ask which paperwork you will need to complete.
If you do not have a printer, you will need to arrive 30 minutes prior to your appointment to complete the necessary paperwork. If your paperwork is not completed prior to your appointment time and you do not show up 30 minutes prior to your appointment to complete the paperwork, you will be rescheduled.
