Women’s Integrated Healthcare, P.A. Visitor Policy Update
We continue to closely monitor guidance on the spread of COVID-19 and are working to ensure that patients get the care they need. We have made the following modifications to our policy, effective April 8 2022.
- A face mask is required (covering your mouth and nose) at all times while in our offices, even in the patient exam rooms. Any person(s) accompanying the patient, will have to adhere to our mask policy as well.
- By entering, visitors acknowledge they have not recently been exposed to a COVID-19 positive individual and are not experiencing any symptoms. If you are experiencing any symptoms, please be courteous and notify our staff.
Thank you for your patience and understanding. We will continue to evaluate our policy as the COVID-19 cases continue to decline.
Please ensure that you arrive with the required paperwork completed upon arrival, if your paperwork is not completed you risk being rescheduled. Please print off the appropriate paperwork for your appointment. You can bring your completed paperwork, upload it to the patient portal, or fax it to Medical Records at 817-416-3667. Please also be prepared to provide your driver’s license/or ID along with your insurance card for scanning and completed paperwork prior to your appointment. We do not accept electronic copies of your insurance cards, if you have an electronic copy, please upload it through the patient portal, PDF versions only accepted through the patient portal.

You were advised while scheduling your appointment which paperwork to complete, if you have forgotten, you will need to contact our office at 817-416-2229, to ask which paperwork you will need to complete.
If you do not have a printer, you will need to arrive 30 minutes prior to your appointment to complete the necessary paperwork. If your paperwork is not completed prior to your appointment time and you do not show up 30 minutes prior to your appointment to complete the paperwork, you will be rescheduled.
