How to complete FMLA and/or Short Term Disability paperwork.

You will need to check with your employer regarding eligibility and forms required for Family Medical Leave and/or Short Term Disability.

Once you have completed your section of your employer’s forms please submit all paperwork to our front desk staff. You will need to allow 14 working days for the forms to be completed. Paternity leave may also be available through the father’s employer. We do charge a fee for the forms to be completed. The fees are as follows:

• 1st set of form(s) – $25.00
• Additional set(s) of forms – $20.00/per set
• Special letters that need to typed/completed – $10.00

Expedite fees for forms are an additional $25.00 if these need to be completed sooner than 14 business days.
